I understand that this Group CGM Program is educational and informational in nature. The program is designed to help participants understand how food, lifestyle, stress, sleep, and movement may impact glucose patterns using CGM data viewed through the Levels app.
Not Medical Care
I acknowledge that Olga Arnold is a Functional Nutritionist, not a physician or other licensed medical provider. This program does not provide medical diagnosis, treatment, prescription, or medical advice, and does not replace care from my physician or healthcare provider.
Medical Oversight Responsibility
I understand that I am solely responsible for managing my medical conditions and medications. I agree not to change, stop, or adjust medications without first consulting my prescribing healthcare provider.
Use of Stelo CGM Device
I understand that the Stelo Continuous Glucose Monitor measures interstitial glucose, not blood glucose, and values may differ from finger-stick readings. CGM data may lag behind blood glucose levels and should not be used for medical emergencies or treatment decisions.
Levels App Data Interpretation
I understand that the Levels app provides data visualization and insights to support learning and awareness, not medical recommendations. Insights and trends shown in the app are not intended to diagnose or treat any condition.
No Guaranteed Outcomes
I acknowledge that individual responses to nutrition and lifestyle changes vary. No specific results—including glucose control, weight loss, symptom improvement, or health outcomes—are guaranteed.
Group Program Environment
I understand this is a group-based program. While confidentiality is encouraged, Olga’s Nutrition, LLC cannot guarantee the behavior or disclosures of other participants.
Voluntary Participation & Assumption of Risk
I voluntarily choose to participate in this program and assume full responsibility for my use of the Stelo CGM, the Levels app, and any nutrition or lifestyle changes I choose to make.
Release of Liability
I release and hold harmless Olga Arnold and Olga’s Nutrition, LLC from any claims, injuries, losses, or damages arising from participation in this program, except in cases of gross negligence.
